Shika 4.0.2 maintenance release

Bugs Fixed (11):
- After editing the course image disappears.
- Lessons: images are not saved.
- Fixed UI issue on lesson format video.
- Missing lessons categories submenu.
- Missing course fields submenu.
- Error on reminder and notification templates.
- Unable to access course details: Class "JRequest" not found.
- Wrong URL when multiple course menus are enabled.
- Content list showing null in reminder setting on the admin side
- Tags are not getting saved.
- Click on Enroll User button without selecting the course and getting an error.
