I really need help ASAP please

I am trying to launch a donation page for my disability therapy centre but my donation page is looking like this - http://223.27.20.128/~move4276/index.php/component/jgive/donations?layout=paymentform and the form doesn't work properly.

I placed a support ticket last night but I haven't had a response.

Can someone please help me fix it? I am not very computer savvy but the longer I can't use my donation page, the less money I am able to raise for my cause.
