Doesn't work for Joomla 1.7 with CB 1.7.1

Like the posters before I am also having issues with this module. I have been using this mod for years on my Joomla 1.5 with CB 1.7.1 with absolutely NO problems what so ever and even added additional fields.

However when i go to configure this mod there are NO parameters.
I am more than willing to take a screenshot of this.

Here are my install details:
Joomla 1.7.5
CB 1.7.1
Kunena 1.6.5
EasyBlog 3.x
GCalendar 2.7
JoomGallery 2.0
Mosets Tree 2.2.3
AlfContact 2.0.3
